Right at Home Care Professionals at Park Plaza in Chicago, IL

Right at Home is the exclusive provider of Shared Care services to residents at Park Plaza, a Jewish Senior Living Community, in Chicago. Shared Care aide service is currently available from 7:00 AM to 8:30 PM daily, but assistance for two or more consecutive hours is available through Right at Home 24 hours per day. A team of five nurses aides cover resident needs with as many as three on site simultaneously.

What is Shared Care?

Shared Care is a la carte services offered by the Right at Home team at Park Plaza to allow residents to receive care in increments of as little as 15 minutes or less at a time. These services are customized to the unique needs of the resident. Longer consecutive hour shifts are also available with coordination through the Care Management department at Right at Home.

Shared Care Services can include:

Bathing and Dressing assistance
Escorts to and from meals and/or activities
Laundry
Medication Reminders
Safety Checks
Emergency Response
Transportation
Garbage Removal
